Breast Augmentation
Breast augmentation can be an excellent surgical option for increasing the size and fullness of the breasts; enhancing the shape and curvature of the breasts; and/or improving breast asymmetries. At our practice, Dr. Broumand can perform breast augmentation using breast implants, fat transfer (natural breast enhancement), or a combination of both techniques.
Breast Augmentation with Implants
For breast implant surgery, incisions are generally made either at the base of the areola or at the underarm crease. The breast implant of choice (saline, traditional silicone, or cohesive gel) is inserted through the incision and placed above or below the breast muscle, depending on the individual’s unique needs. Once the implants are situated properly, the incisions are closed with tiny sutures. On average, breast augmentation with implants takes approximately an hour to complete, and the majority of patients are able to resume normal, non-strenuous activities and daily routines after about a week.
Natural Breast Enhancement with Fat Transfer
For natural breast enhancement, a tissue expansion system known as BRAVA® is employed prior to the procedure in order to create an ideal environment for fat transfer. Once the tissue expansion process is complete, the natural breast enhancement procedure involves the harvesting of fat from a donor area of the body via liposuction, followed by the cleansing and reinjection of collected fat into the breasts until optimal outcomes are achieved. The entire procedure generally takes about two hours to complete, and most individuals are able to return to non-strenuous work and daily routines after approximately ten days.
Breast Lift
Medically referred to as mastopexy, breast lift surgery can raise the breasts to correct sagging/drooping; tighten and remove loose, excess breast skin; recontour breast tissue for an improved shape; elevate the positioning of the nipple-areolar complex; and/or reduce the size of the areola. During mastopexy, a “lollipop” incision is generally made around the areola and extending down the underside of the breasts to the inframammary crease. Excess skin is then excised, and breast tissues are reshaped and lifted until the desired contour and elevation is achieved. Subsequently, the remaining skin is reattached and carefully sutured.
The entire breast lift procedure can typically be completed in about one to two hours, and most patients feel well enough to return to their normal daily routines within one week. For individuals also interested in increasing the size of their breasts, a breast augmentation can often be performed in combination with mastopexy.
Breast Lift with Implants
Also called augmentation-mastopexy or breast augmentation with a lift, a breast lift with implants can be an excellent choice for many patients seeking to comprehensively rejuvenate the appearance of their breasts. This combination treatment can help enhance breast size; restore lost volume; correct breast sagging/drooping; raise the breasts to a higher location on the chest wall; and improve the overall shape and symmetry of the breasts. During the procedure, excess skin is removed and breast tissues are lifted to an ideal position. The selected breast implants are then carefully inserted and secured in place, and the skin is pulled tight and reattached to create an optimal breast contour.
A breast lift with implants can generally be completed in about two to three hours, and the majority of women are able to resume non-strenuous jobs and activities within seven days. In the rare case surgery needs to be conducted in stages, each recovery period will be similar to that of mastopexy and breast augmentation performed on their own.
Breast Reduction
For women whose breasts are disproportionately large for their body frame, breast reduction surgery can often be a great option. This procedure can generally help alleviate chronic pain in the back, neck, and shoulders; persistent headaches extending from the back of the neck to the forehead; tender bra strap indentations; painful skin rashes under the breasts; and/or poor posture caused by the weight of excessive breast tissue. During breast reduction surgery, a “lollipop” incision that encircles the areola and extends down to the inframammary fold is typically employed. Excess breast tissue, fat, and skin is removed as needed, and the remaining skin and tissues are raised and reshaped for an optimal outcome.
Based on the patient’s needs, the entire breast reduction procedure typically takes about two hours to complete. Following surgery, most individuals feel well enough to resume non-strenuous activities after approximately one week.
Breast Revision Surgery
Breast revision surgery may be an excellent procedural option for patients who are not happy with the results of a previous breast operation, individuals whose breast enhancement goals have changed over time, and/or those who have encountered an issue from a past procedure. Some of the most common reasons to undergo breast revision surgery include a desire to upgrade from saline to silicone implants; increase/decrease the breast size; enhance the shape or symmetry of the breasts; replace a ruptured or leaking implant; and/or correct implant complications such as capsular contracture, malpositioning, rippling, and more. During this breast procedure, the original incision pattern is generally used to remove the current implant and replace it with a new one (if desired). The new implant will be positioned appropriately for ideal outcomes, which may involve a new pocket location, depending on whether or not the former implants were situated above or below the muscle.
For most patients, breast revision surgery can be completed in about an hour and a half, though this will vary based on the complexity of the operation. Following surgery, the majority of individuals feel well enough to resume normal daily routines and non-strenuous activities within one week.
Breast Asymmetry
Whether the differences are practically unperceivable or clearly evident, no two natural breasts are perfectly symmetrical. Most breast asymmetries form when the breasts are developing, though factors like genetics, pregnancy/breastfeeding, weight fluctuations, and aging can all contribute to a more pronounced appearance. Dr. Broumand offers several treatment options for patients bothered by breast asymmetry, including augmentation or reduction of one or both breasts, as well as breast lift surgery for one or both sides. They can also revise the nipple-areolar complex(es) alone or in combination with another procedure. By custom-tailoring treatment to meet each patient’s specific needs, our doctor can help balance the shape, size, and position of the breasts for a beautiful, symmetrical appearance.
Depending on the particular procedure(s) employed, breast asymmetry treatment may take anywhere from one to three hours to complete. Following surgery, most women are able to return to normal, non-strenuous daily activities within approximately one week.
Breast Reconstruction
For women who have undergone (or are considering) mastectomy as part of their fight to defeat breast cancer, breast reconstruction surgery can often be an excellent option to help restore the breasts to a normal shape, size, and overall appearance. At 740 Park Plastic Surgery, our doctor offers several different techniques, including implant reconstruction, fat grafting, and DIEP flap breast reconstruction. In some instances, a nipple sparing mastectomy can be performed prior to breast reconstruction in order to help preserve the nipple-areolar complex.
Implant Breast Reconstruction
Prior to breast reconstruction with implants, a tissue expander is used to create a breast pocket large enough to safely and effectively accommodate the patient’s implant of choice. Once enough room has been created, the breast implant(s) will be placed and positioned using limited incisions for an optimal result. If desired, fat grafting can often be employed to finely shape the breast for an enhanced outcome.
Breast Reconstruction with Fat Grafting
Similar to natural breast enhancement, breast reconstruction with fat grafting involves the harvesting and transfer of fatty tissues to create a new breast mound. Prior to the procedure, the BRAVA® tissue expansion system is used to create an optimal environment for the transfer of fat to the breasts. Once this process is complete, fat is harvested via liposuction from one or more areas of the body that contain an ample amount of fat accumulation. The fat is then purified and reinjected into the breast(s) until an ideal outcome is achieved.
DIEP Flap Breast Reconstruction
The DIEP (deep inferior epigastric perforator) flap technique for breast reconstruction uses tissues taken from the patient’s lower abdomen to rebuild the breast(s). During this procedure, fat and abdominal tissues are extracted and relocated to the chest wall, and small blood vessels are connected to enable sufficient blood supply. The tissues are then carefully sculpted to form the new breast shape, and the skin of the abdomen is redraped and tightened for a more toned aesthetic.
The length of the procedure as well as the recovery timeframe for breast reconstruction will depend on the particular technique being employed. On average, the majority of patients are able to return to normal, non-strenuous daily activities within one to two weeks.
Breast Implant Removal
Explant surgery, also known as breast implant removal, is a procedure undertaken to remove saline or silicone implants from the breast. Patients may opt for removal due to various reasons, including implant rupture, capsular contracture (where scar tissue forms around the implant, causing discomfort), dissatisfaction with size or appearance, or health issues. The surgery can restore the breasts to their natural state or involve replacing the implants and can provide relief from symptoms associated with previous breast surgery.
Gynecomastia (Male Breast Reduction)
Gynecomastia is a common condition characterized by the enlargement of male breasts due to excess accumulations of glandular tissue and/or fat. For men interested in treating gynecomastia, male breast reduction surgery can often be an excellent option. At 740 Park Plastic Surgery, this procedure is customized to remove excess glandular tissue; eliminate buildups of fatty tissue in the breasts; excise loose, redundant breast skin; and/or reposition or diminish the size of the nipple-areola complex. During male breast reduction, glandular tissue is removed through micro-incisions made inconspicuously around the areola. If necessary, liposuction is also employed through tiny incisions made at the lower chest to remove fat accumulations until the desired outcome is achieved.
Depending on the patient’s needs, the entire procedure takes approximately one to two hours to complete, and most individuals are able to resume work and non-strenuous activities within a few days to a week after treatment.
